Dear Parents,
Please find below links to two forms pertaining to the All Catholic Conference, our Archdiocesan, elementary school, athletic association. We ask that you have both forms completed each year, after May 15 or before the 1st day of tryout. Only after these forms have been submitted is your child permitted to participate in the athletic events conducted by the All Catholic Conference.
The Athletic Consent and Release from Liability Certificate must be completed by the student's parent or guardian and the Athletic Pre-participation Physical Evaluation form must be completed by a licensed physician (MD or DO). Please note that the Athletic Pre-participation Physical Evaluation form will be used ONLY for the purpose of determining a student's athletic eligibility.
In addition, our school's ability to provide extra-curricular sports programs depends on the financial support of the participants. There is a mandatory $30.00 sports fee for every participant on every team. Also participants must pay for their own uniforms. Students will not be placed on the team roster until the sports fee and uniform expense are paid.
